In short
A hash is data's digital fingerprint: every input produces a unique, fixed string. Change one bit and the hash changes completely.
Blockchains chain blocks via hashes: each block contains the previous one's hash. Any later change is instantly visible — the chain "breaks".
Can a hash be reversed?
Practically no — exactly this one-way property makes it useful as a fingerprint.
